Position is located in Education.
Incumbent is responsible for the documented outcomes at the program, service and facility level. Program, service and facility level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, VISN, and/or VACO. Program is defined as a substantial coordinated group of activities impacting patients and/or employees.
Competencies include planning and organizing the work, estimating costs and requirements, coordinating with staff and line supervisors' findings, issues or problems. Completed projects, evaluations, reports or recommendations are reviewed by the supervisor for compatibility with overall organizational goals, guidelines and effectiveness in achieving the intended objectives. Designs, implements and evaluates patient-specific and population based-programs. Influences care outcomes by providing expert consultation and guidance to nursing staff within the field of SPH and fall prevention by implementing improvements in health care delivery systems. Assesses, plans, implements and evaluates the total health care of a specialized patient population in a clinical specialty area.
Resolves unusual or unique patient care problems or situations which requires expertise related to falls and SPH. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility and is accountable for providing assistance in various aspects of the SPH and movement program incorporating a "NO LIFT" status at VACCHCS. Included are developing, implementing, maintaining, and continuing evaluation of the hospital's program for safe patient handing policies, procedures for both new and current staff.
Responsible for assisting in managing and coordinating all patient care, supervision, coordination and expansion of the unit peer leaders Develops resources in other services/disciplines; Implements SPH algorithms; Identifies, proposes and provides oversight of equipment selection and purchase to meet current and future needs for SPH; and Provides oversight of facility bariatric issues related to SPH needs.
Work Schedule: Monday - Friday, 7:30 am to 4:00 pm - flexible as needed for training
