This position is located at the Cheyenne VA Medical Center in Cheyenne, Wyoming and functions as the Homeless Program Grant and Per Diem (GPD) Liaison Coordinator (Social Worker) within the Mental Health Service. Duties may include, but are not limited to:
-Provides coordination of care for homeless Veterans by encouraging referrals from community organizations and faith based groups serving the homeless.
-Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the CVAHCS, VA Regional Offices and/or community agencies.
-Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans and GPD Program participants.
-Monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application.
-Provides services and oversight of the GPD funded programs as defined in the GPD rules, regulations, and directives.
-Coordinates and participates in annual GPD and Contract Emergency Residential Services (CERS) program site inspections utilizing the GPD and CERS Programs rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices.
-Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the Homeless GPD Liaison/Coordinator.
-Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
-Provides crisis intervention when indicated including evaluation, assessment, triage, initiation of/support in Emergency Detention procedures within scope of licensure, and disposition of patients experiencing behavioral health emergencies.
-May transport veterans in government vehicles in support of meeting the goals identified in the veteran's treatment plan and/or assisting veteran with access to care.
-Attends Mental Health Staff meetings and any other mandatory meetings regularly.
Work Schedule: Monday - Friday, 8am - 4:30pm
