    The Community Social Worker works within Social Work Service and provides services to Veterans receiving non-institutional and institutional care through the New Mexico VA Health Care System.

    Responsibilities

    He/she is responsible for the visits to Veterans placed in the Community Nursing Home (CNH) Program and may also provide services to Veterans in other community-based programs, such as Community Adult Day HealthCare. Services include provision of high-quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the Veteran. This is accomplished in collaboration with other members of the interdisciplinary treatment team.

    The functions at the full performance level which requires completion of a minimum of 1 year of post-MSW degree experience and licensure or certification in a state at the independent practice level and practices under the NASW Code of Ethics. The Social Worker provides services on an inpatient and outpatient basis.

    Work Schedule: 8:00-4:30, Monday-Friday
