Community Health Nursing Coordinators (CHNC) are responsible for discharge planning and continuity of care for those patients identified as requiring home care services. The Coordinators and the Veteran's health care team work together to provide for the patient's medical, nursing, emotional, social and rehabilitative needs. Our Community Health Nursing Coordinators work as case managers to coordinate care that is provided by home care agencies in the community. Services available within the home setting may include skilled nursing, physical and/or occupational therapy, home hospice, and home health aides for personal care and light housekeeping.
Work Schedule: Full-Time, Monday - Friday, 7:30 am - 4:00 pm
