Physician Assistant/Nurse Practitioner is expected to conduct comprehensive evaluation and management of veterans including review of medical records, patient/caregiver interviews, completion of comprehensive palliative care and geriatrics assessment, creating and implementing treatment plan of care in collaboration with attending physician. Care coordination includes medication reconciliation, medical equipment needs assessment/provision, primary care/specialty clinic follow ups, patient and caregiver education.
Work Schedule: Mon-Fri 8am-4:30pm with possible weekend/on-call coverage

Physical Requirements: The Physical requirements of this position include the following tasks which may be performed intermittently: Performs direct patient care and Lift 45 lbs. or over, carry 45 lbs. or over, straight pulling, Pushing, Reaching above shoulders, use of fingers and both hands, Walking, Standing, Kneeling, Repeated bending, Near vision (correctable), Far vision (correctable), Ability to distinguish basic colors, Ability to distinguish shades of colors, Hearing, and Emotional stability.
This Physicians Assistant (not, Nurse Practitioner) position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program within VHA. If selected for this position, you are encouraged to apply for this award program and must meet specific individual eligibility criteria in accordance with VA Policy 1021. Employees must apply for EDRP within four (4) months of appointment. Final award amount and program start date is determined after review of a complete EDRP application and supporting documentation by the VHA Education Loan Repayment Services team. Maximum award is total amount of qualifying loans up to $200,000. If selected, the local HR POC will provide the EDRP contact information for application materials and procedures.
