    This position leads the Community Care Section within Health Administration Service (HAS) at the Battle Creek VA Medical Center (BCVAMC) and reports to the Chief of HAS. The Chief of Community Care will utilize the nursing process and evidence-based practice to collaborate with staff at the BCVAMC and other health care facilities involved in providing care to our Veterans and promoting Personalized, Proactive, Patient Driven care.

    The Chief, Community Care manages all aspects of the program including contacts with external healthcare entities, contacts with clinical bed section and manages a team of 55-65 employees. The incumbent works closely with other services throughout the medical center and facility leaders to support the daily operations of Community Care Coordination within the 26-county catchment area the Battle Creek Delivery System serves.

    The Chief of Community Care provides executive leadership and direct supervision for the Community Care Section within HAS. This Service works directly for the Associate Director while partner with the Chief of Staff and Associate Director for Patient Care Services.

    The primary objective of the Community Care Section is to centralize all Non-VA Care for the entire Battle Creek VA Medical Center. The role includes facilitating patient care through Non-VA Care Access for the traditional Non-VA Care, VA Choice and the VA MISSION Act. Success of this initiative is dependent on the transitional plan to a centralized unit, resource allocation and the willingness to work as a team.

    Preferred Experience: The preferred candidate will possess a MSN and have five years of progressive leadership and case management experience in a hospital or clinical environment.
    Work Schedule: Monday through Friday, 8:00AM-4:30PM or 7:30AM-4:00PM. The nature of the role will require being available for extended hours and emergent situations on evenings and weekends, based upon the needs of our Veterans and the Medical Center.
