There is a full time opening for a Nurse Practitioner (NP) in Primary Care as a Gap Provider within the Northeast Wisconsin Ambulatory Care Clinics-Appleton, Cleveland and Green Bay. Responsibilities include a collaborative practice within our primary care clinics, with oversight of a designated panel of veteran patients. The NP would provide coverage while another provider is on leave or for urgent care issues as needed.
This position will provide coverage at any of our facilities within the Northeast Wisconsin Ambulatory Care Division (Green Bay Health Care Center, Appleton Community Based Clinic, or Cleveland Community Based Clinic). The primary care Gap Provider will be responsible for a panel of up to 1200 patients and a workload on average per day of up to 16 patients. The primary care Gap Provider is a member of a Patient Aligned Care Team (PharmD, RD, SW, and Nursing) that is responsible for managing the health prevention/disease management needs of the assigned panel of patients, including urgent care. The incumbent will also be responsible to complete clinical reminders, achieve clinical performance measures, medication reconciliation and be competent with the computerized patient record system (CPRS).
The focus of Primary Care at the Northeast Wisconsin Ambulatory Clinics is the provision of general medical care for veteran patients in a manner which is reflective of good clinical judgment, medical knowledge, clinical skills (medical interviewing, physical examination, and procedural skills), humanistic qualities and professionalism.
Work Schedule: Day Shift, Monday through Friday with potential for extended hours during the weekday or weekends
