The incumbent collaborates with CVAMC leadership to ensure alignment of geriatric and extended care programs, policies, and resources are consistent with the facility's mission, goals and performance measures. Promotes and supports Shared Governance objectives and Patient Centered Care. Prepares staff for Long Term Care Institute (LTCI) annual surveys. Provides direct oversight and is directly responsible for all aspects of nursing care and documentation delivered to the following programs and services: 148 bed Community Living Centers (CLC), Home Based Primary Care (HBPC), Hospice and Palliative Care services, Non-Institutional Care Programs (NIC), State Veterans Home Program, Contract Nursing Home Programs, and Medical Foster Home Program.
The Director for Geriatrics & Extended Care Services reports to the Associate Director for Patient Care Services/Nurse Executive. This position assesses, plans and evaluates the processes and daily operations to design and implement an interdisciplinary program. The incumbent provides executive leadership and direct supervision to Nurse Managers, Program Leaders and support staff. The incumbent is responsible for assessing and recommending allocation of resources utilizing staffing methodology and LEAN management. The incumbent has a strong impact on the recruitment and retention of nursing staff, and engaging staff in planning for improvements in operations.

Reporting Structure: Reports to the Associate Director, Patient Care Services.
Tour of Duty: Day Tour working Monday - Friday with occasional off-tour, if needed
