CLC short stay and post-acute care program
The Nurse Practitioner (NP) for the Geriatrics & Extended Care Service will serve as a member of an interdisciplinary team in Community Living Center (CLC) short stay program- post acute care. The NP is responsible for wide range primary care services for patient-family care for assigned panel of patients in the CLC short stay-post acute care program. These patients have identified needs that require CLC level care such as short stay rehab, short stay skilled care (complex wound care, IV antibiotics, etc.), short stay dementia care, short stay mental health recovery, and post-operative skilled and rehabilitation services.
Duties include, but are not limited to:
-Admission screening, Admission History and Physical, daily rounds, write orders, participation in daily team huddles, wound care and interdisciplinary team meetings.
-Collaborate to initiate and implement interdisciplinary care plan, write progress notes, initiate specialty care & rehabilitation consults to implement care plan, timely completion of discharge orders/summaries.
-Collaborate with facility wide treatment teams to implement care plan and facilitate discharge/ transition of care.
-Review and revise patient care based on clinical findings, specialty care consultation and assessment while following approved clinical protocols/privileges.
-Initiate & interpret diagnostic tests as appropriate based of clinical privileges and scope of practice.
-Collaborates with managing physician daily on all aspects of patient-family care.-Provide patient-family education.
-Provide education to CLC interdisciplinary team members.
-Participate in MDS, quality measures and quality improvement projects.
-Responsible and accountable for maintaining National Certification in advanced practice nursing and prescriptive authority and maintain BLS.
-Responsible for demonstrating the educational, and knowledge/practice-based competency requirements outlined in the four (4) dimensions: Practice, Professional Development, Collaboration, Scientific Inquiry.
