This position is located in the Primary Care service of the Stratton VA Medical Center. If selected, you will practice at the Clifton Park, NY Community Based Outpatient Clinic. You will function as an integral member of the Patient-Aligned Care Team and will be expected to work closely with team members, including nursing and administrative staff, in the coordination of patient care. You will participate in daily huddles with team members and in primary care systems redesign projects.

You will be assigned a panel of patients to manage under the Department of Veterans Affairs Primary Care Management Model guidelines. Recommended panel size at the division level is calculated based on patient complexity, support staff, and room availability. You will be expected to provide clinical oversight and participate in peer review of other primary care providers through periodic chart reviews.
Your basic responsibilities will include:
· Providing patient care
· Acting as surrogate for absent providers when needed.
· Attending staff meeting and medical meetings
· Following Veterans Health Administration guidelines
· Timely documentation of encounters
· Working within your scope of practice
Work Schedule: Monday - Friday, 8:00am - 4:30pm

The Stratton VA Medical Center, part of the NY/NJ Healthcare System, is located in the heart of Albany. Opened in 1951, our Medical Center serves 22 counties of Upstate New York, Western Massachusetts and Vermont. In 1990 this facility was named in honor of Samuel S. Education Debt Reduction Program (EDRP): This position is eligible for EDRP, a student loan reimbursement program.
