    The RN Care Manager uses the nursing process and evidence-based practice to collaborate with all members of the Eye Care Team, including ophthalmologists, optometrist, eye technicians, low vision, blind rehab staff, opticians, volunteers, and BDMS. Typical duties include: Triage all incoming consults. Run all BDMS reports regarding consults and access measures. Participate in the assessment and management of access and wait times for the Eye Section.

    Manage the vesting process and assist with closure of all open encounters. Manage the intra-ocular eye injection program, start IV for eye angiograms, and order medications. Manage eye surgical schedule and schedule surgeries with the OR scheduler. Check charts to assure the patients are ready for surgery and have had their pre-op evaluation. Telephone the patients with the date and time of their surgery. Work closely with the eye technicians and BDMS staff. Serves as a backup to the eye technician in their technical and other patient care duties in performance of patient check in, visual acuity, OCT testing, intraocular pressure measurements, dilation, visual fields, triage of walk in patients and other technical duties as needed. Review clinic schedules and help manage the clinic cancellations. Coordinate referrals of patients to and from other VA facilities, coordinate fee basis patients to the University of California at Davis or the community. Fee out optometry patients and manage the Wal Mart program for eye exams. Help case manage no show and non-compliant patients. Support similar nurse care manager functions as previously listed and as needed for the Oakland/East Bay Eye Clinic. Assist in coordination of quarterly M&M conference. Other duties as needed.
