The incumbent is the medical nutrition expert responsible for the independent planning, development and directing of all nutritional care for assigned Veterans, including those with complex medical and nutritional needs. The incumbent will have a skilled knowledge base to formulate clinical assessments and provide dietetic support to Veterans and their support system. The incumbent supports the facility's mission and coordinates nutritional care with the Veteran, the Veteran's family, other health care providers, and the community. The incumbent will have a wide range of disease entities, treatment modalities and drug/nutrient interactions.
The incumbent will have the ability to expand clinical knowledge in the profession, provide consultation and guidance to colleagues, role model effective work practice skills, and teach or provide orientation to less experienced staff members. He/she will assist in developing innovations to improve practice interventions. The incumbent will need to drive a car and have a valid state license.
The incumbent will be a member of Home Based Primary Care (HBPC) providing comprehensive, interdisciplinary primary care services in the homes of Veterans with complex, chronic, and disabling disease. In additional to primary care interventions, HBPC provides palliative care, rehabilitation, disease management, and care coordination services. HBPC targets Veterans in advanced stages of chronic disease, particularly those at elevated risk for recurrent hospitalization or nursing home placement. Outcomes expected of HBPC include effective management of chronic disease and reduction of inpatient days and total cost of care. HBPC teams include representatives from medicine service, nursing, pharmacy, social work, rehabilitation, dietetics and psychology. The incumbent is to engage the Veteran and his/her support system in a therapeutic relationship to facilitate optimal level of functioning in a home care setting. The incumbent serves as the subject matter expert in medical dietetics. The incumbent will possess effective communication skills and participate as an active member of the HBPC Interdisciplinary Treatment Team (IDTT). The incumbent is assigned full-time HBPC for the West Texas VA Health Care System (WTVAHCS) with an association with Nutrition and Food Service. The incumbent may be scheduled or assigned to any nutrition area.
