    Summary

    This position is located within the Primary Care Service line as part of a Patient-Aligned Care Team (PACT). The Licensed Practical Nurse (LPN) works under the direction of an RN and reports directly to the Primary Care Nurse Manager. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the implementation of the patient-driven holistic care plan for life.

    Responsibilities

    • Provides support for the Patient Aligned Care Team in the Primary Care clinics following established nursing and primary care policies and procedures.
    • Demonstrates age-appropriate care for those whom may be experiencing medical and psychological consequences of illness (young adulthood through geriatric).
    • Provides a safe environment as demonstrated by compliance with sound safety practices following VA CWM HCS regulations.
    • Participates in selected health screenings, clinical reminders, telephone calls, and nursing treatments as assigned.
    • Supports a patient focused environment through courteousness and respectful dealings with patients, families, and coworkers.
    • Ensures that all outpatient clinic appointments are scheduled in accordance with the business rules in VHA Directive Out-Patient Scheduling Processes and Procedures.

    Work Schedule: 7:30am - 4:00pm, Mon - Fri
