    The Geriatrics and Extended Care (GEC) Program encompasses a spectrum of patient care services in support of the long-term chronically ill and elderly veteran who requires services throughout the continuum of care. The GEC/Home-Based Primary Care (HBPC) Staff Physician plays a vital role in enhancing and coordinating the clinical care of veterans in our GEC and Palliative Care programs at various clinical sites.

    The GEC/HBPC Staff Physician:

    - Provides direct medical care to veterans including assessment of medical conditions, writing orders, medication monitoring, interpretation of diagnostic tests, formulation of treatment plans, accessing VA and community services, participating in interdisciplinary team care
    - Participates in developing policies, procedures, and guidelines for comprehensive services which meet or exceed industry standards
    - Participates in the on call, transfers, and emergency treatment procedures for veterans in all sites of clinical GEC/Palliative programs
    - Participates in developing and recommending transitions of care for veterans
    - Collaborates with Community Living Center (CLC), Palliative Care, HBPC, HiH, and medical directors regarding clinical services and adequacy of equipment, professional and support staff at sites of clinical care
    - Participates in on-call responsibilities and in-service training programs for GEC services
    - Serves as a member of the organized medical staff, participates in staff meetings and ensures adherence to medical center policies, bylaws, rules and regulations
    - Ensures that performance objectives established by VACO, VISN, local facility are met
    - Ensures that clinical protocols, utilization review, case management; quality assurance and compliance programs are followed and feedback for improvement provided
    - Ensures compliance w/facility policy for timely completion of progress notes, consults, discharge summaries, and encounters
    - Creates and participates in educational activities including continuing medical education programs
    - Provides direct supervision and participates in the education of medical/ geriatric/palliative care trainees and other professional staff
    - Participates in GEC in quality improvement projects and initiates projects as they become identified
    - Actively participates with Geriatric and Palliative Care academic partners
    - Duties as assigned
