The incumbent will be responsible for the management and coordination of Community Homemaker/Home Healthcare services and in-home respite services.
The incumbent will be fully responsible for the coordination of the Community Homemaker/ Home HealthCare Services and In-Home Respite Services. The H/HHA Coordinator in conjunction with the Program Manager, GEC Lead Social Worker plans, organizes and directs administrative, operational, and patient activities for the programs. The incumbent acts as the liaison between the community agency, Veterans and VA in an effort to resolve any psychosocial problems and/ or complex health needs which require and administrative decision.
Work Schedule: MONDAY - FRIDAY 8:00 AM - 4:30 PM
