An opportunity exists for a full-time GAP Physician. This Physician will provide primary care of a panel of veterans at the Appleton VA Community-Based Outpatient Clinic in Appleton, WI as the covering physician when another provider is absent. At times, this may include working in this capacity at the community-based outpatient clinics in Green Bay and Cleveland, WI. The primary care physician will be responsible for a panel of up to 1200 patients and a workload on average per day of 16 patients. The primary care physician is a member of a Patient Aligned Care Team (PharmD, RD, SW, and Nursing) that are responsible for managing the health prevention/disease management needs of the assigned panel of patients. The incumbent will also be responsible to complete clinical reminders, achieve clinical performance measures, medication reconciliation and be competent with the computerized patient record system (CPRS).
The focus of a Primary Care physician at the Appleton VA CBOC is the provision of general medical care and long term care for veteran patients in a manner which is reflective of good clinical judgment, medical knowledge, clinical skills (medical interviewing, physical examination, and procedural skills), humanistic qualities and professionalism.
Work Schedule: Monday through Friday, 8:00AM-4:30PM
