The incumbent practices as a registered nurse in an Outpatient Behavioral Health Care setting. This is an Open Continuous Announcement. Application packages for potential vacancies in our Mental Health unit will be maintained and used when vacancies arise; positions will be filled with qualified applicants. **The first list of eligible applicants will be pulled for referral on Thursday, March 21, 2019*. Subsequent referrals of qualified applicants will be referred on the 22nd of each month (Cutoff dates) or as needed. The vacancy announcement will remain open until closed, canceled or filled.**
All outpatient mental health settings require the same basic nursing functions:
- Assessment of veterans, surroundings, program component and surroundings. Planning for veteran care, program activities and services. Interventions initiated to assist the veteran to achieve the highest level of functioning possible and to meet program, medical center and regulatory goals. Evaluation of care provided and progress towards goals and analyzes and upgrades patient care though quality improvement activities. Specializes in treating adults and geriatrics veterans who are the most severely disabled psychiatric patients. The most common diagnosis is schizophrenia, many veterans have dual diagnosis. Provides leadership in the application of the nursing process to client care, organizational processes and/or systems, improving outcomes at the program or service level. Performs complex/comprehensive physical, psychosocial and spiritual assessments. Observes and interprets patterns of patient behavior and/or problems in operational systems or processes. Disseminates complex patient/family symptoms and needs and/or systems issues during assessment. Identifies intricate patient /systems problems and accurately focuses on these problem areas. Determines desired outcomes consistent with patient care (and/or systems/operations); issues and institutes a plan of care/action to achieve outcomes. Demonstrates applicable knowledge of technical skills such as but not limited to blood glucose monitoring, electronic vital signs machines, using the computerized medical record etc. Provide medication management, review outpatient medications and evaluate interventions from treatment plan. Complete home visits, transports patients, supplies and groups. Provide patient interactions in the community. Provide nursing services in a residential setting using the recovery model for patients at the Domiciliary.
