    The VA Ann Arbor Healthcare System is seeking a Licensed Practical Nurse to be a member of our staff at the Toledo Community Based Outpatient Clinic.

    The Licensed Practical/Vocational Nurse works under the direction of a RN and reports to the Nurse Manager. The incumbent is responsible for observing, recording and reporting changes in the patient's condition, assisting in the development and revision of the patient-driven holistic care plan for life; requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures; makes observations and thoroughly documents care and the patient's response; and seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention

    The LPN performs, reports, and records the assigned nursing care in the Toledo Community Based Outpatient Clinic (CBOC) including but not limited to:

    • Vital Signs including pulse, blood pressure, respirations, temperature, pain level, and obtaining pulse oximetry
    • Administers influenza, pneumococcal and tetanus vaccinations
    • Refers patients to appropriate resources including social work, dietitian, speech pathology, SAC, smoking cessation and physical and occupational therapy after screening
    • Glucometer measurements
    • Neurological vital signs
    • Recognizes urgent/emergent patient care situations and initiates assistance effectively intervening as appropriate
    • Provides patient/family education reinforces health teaching and takes an active role in promoting healthy lifestyles
    • Maintains precise adherence to basic principles of asepsis, safety, and universal precautions to provide a safe environment for patients, families, and employees
    • Incumbent possesses knowledge of proper handling of intruments including the cleaning of reusable medical equipment
    • Utilizes special equipment such as IV infusion pumps, Doppler units, pulse oximetry, dermatology instruments needed to perform job duties including computer terminal and printer
    • Provides 1:1 nursing care to assigned outpatients whose care requires close observation during procedures
    • Follows policy and procedures related to fire and disaster drills, safety and security measures
    • Interprets Medical Center Regulations and policies to other staff, patients, relatives, and visitors
    • Proactively assists the medical provider and/or nurse with therapeutic or diagnostic procedures, including biopsies and minor surgeries
    Work Schedule: Monday - Friday, 7:30 a.m. to 4:00 p.m., Occasional evening/weekend possible
