VA Palo Alto Health Care System (VAPAHCS) has immediate openings for THREE (3) Ambulatory Care Clinical Pharmacy Specialists (CPS); one to work from the Stockton Clinic, and one to work from the Modesto Clinic, and one to work from the Palo Alto campus. These pharmacists perform clinical functions independently and maintain a scope of practice in specific therapeutic disease states (including, but not limited to: Diabetes, Hypertension, Hyperlipidemia, Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, first step Pain management, Anticoagulation). These pharmacists are integral members of the Patient Aligned Care Teams (PACT) and work collaboratively with the interdisciplinary team.
