    Summary

    The Southeast Louisiana Veterans Health Care System provides quality, patient-centered health care to Veterans throughout 23 parishes in southeast Louisiana. We meet the needs of Veterans through seven community-based outpatient clinics located in Slidell, Hammond, St. John Parish, Houma, Franklin, Bogalusa and Baton Rouge, as well as a new Veterans Medical Center in New Orleans.     Responsibilities

    The Transition and Care Management (TCM) Nurses provide patient-centered, interdisciplinary health care services to all Post -9/11 active duty service members and Veterans. Per VHA Directive, care provided to these Veterans is coordinated and tracked. All Post-9/11 service members and Veterans are screened for case management services to ensure a smooth transition from military to civilian life, and into the Veterans Administration (VA) health care system. The nurse will provide education and risk factor assessments. The TCM Program provides services to patients and family members by referral. Care management services are provided to ensure patients receive appropriate, quality and timely care across the continuum, ensuring an interdisciplinary approach is used to address medical, psychosocial and socioeconomic needs, and provide appropriate resources. Case management is provided at the following levels, with the majority being at the supportive level. Levels of Case Management are: Intensive Case Management (weekly contact), Progressive Case Management (monthly or more often), Supportive Case Management (quarterly), and Lifetime Case Management (annually).

    Work Schedule: M-F 0730-1600, Evenings,/weekends and/or as patient care dictates.
