    Work Schedule: Full-Time

    Incumbent is the Program Coordinator for the Mental Health Intensive Case Management (MHICM) program, a community based case management program based on the Assertive Community Treatment model. Incumbent both coordinates this multidisciplinary program and directly supervises the Social Work, Peer Specialist and administrative staff that work in this program. In addition, incumbent provides leadership to the Veteran's Recovery Center (VRC). All of these programs are grounded in psychosocial rehabilitation and treatment with particular emphasis in services for veterans with Serious Mental Illness. Incumbent may provide technical and clinical supervision for all staff in these two programs. The goal of these programs is to work collaboratively to enhance veteran wellness, recovery, and independence; enhance the effective utilization of both VA and community resources; provide direct treatment for veterans; respond to or prevent crisis situations threatening the stability of veterans living in the community; and minimize unnecessary utilization of institutional levels of care. The programs utilize multi-disciplinary staff, including Nursing, Social Work, Recreation Therapy, Peer Support and other administrative support staff. The Program Coordinator actively collaborates with mental health programs across the continuum of care and is a key stakeholder and champion for recovery oriented services.

    The major duties and responsibilities include, but are not limited to:

    • Oversees programs providing case management, psychosocial rehabilitation, and social work services. These programs are based in multiple sites.
    • Develops and evaluates programs and services for continuous improvement and/or to meet national guidelines and veteran needs.
    • Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
