Duties include but are not limited to:
This position is part of the team of Home Based Primary Care program. The position is for working in the community setting (patient's home), thus applicant must be willing to drive to cover areas for patient care as needed. Completes initial assessments and reassessments of patient's condition and response to treatment and other clinical findings.Uses sound clinical judgment in assessing clinical changes in patient condition, providing a comprehensive assessment to the provider to facilitate the provider's decision making to effectively manage the patient condition. Initiates and/or recommends laboratory tests, diagnostic studies and/or specialty referrals based on protocols, clinical practice guidelines and/or standards of care.Provides age-specific care that is appropriate to the cognitive, emotional, cultural and chronological maturation needs of the patients.Applies knowledge of diverse nursing procedures/protocols/etc. and takes a leadership role in using safe, cost effective measures to provide care.Effectively participates in the interdisciplinary care planning meetings a assuring the flow of patient care and resources based on the patient's care plan.Identifies patient care needs for support services, education and other resources and acts through referrals, consults, and direct actions to ensure needs are met.Implements an educational plan to meet changing program or service needs for self and others. Maintain knowledge of current techniques, tends and professional issues.Demonstrates knowledge in skilled nursing care, rehabilitation, restorative care, long term spinal cord injury and diseases and hospice and palliative care.Participates in the Geriatrics & Extended Care education program and continuing education activities.Uses professional standards of care and practice to evaluate programs and/or service activities.Demonstrates positive communication skills and behaviors that promote cooperation and teamwork with internal and external customers.Collaborates with other disciplines and caregivers, using group processes for decision making in interdisciplinary problem solving.Makes sustained contributions to healthcare by sharing expertise within and/or outside the medical facility.Initiates interdisciplinary projects to improve organizational performance.Collaborates with others in research activities to improve care.Demonstrates knowledge of quality improvement techniques and performance improvement process.
Work Schedule: Monday to Friday from 8:00 am - 4:30 pm
