Duties include providing direct patient care to assigned clinic panel within a patient care aligned team (PACT). The Nurse Practitioners will execute position responsibilities that demonstrate leadership, experience and creative approaches to management of complex client care for Veterans receiving care through the Primary Care Clinics. Responsibilities include improving patient care, maintaining standards of care and directing and teaching personnel in a variety of settings. This position includes the management of required labs, medications, consultation and/or follow-up planning. Progress notes and orders are required on all patients, as well as histories and physicals when indicated. Care planning with collaborating physicians, peers, patients, families and allied services is expected. Professional activities include PACT team management, committee involvement, professional lectures and process improvement activities.
Work Schedule: The tour of duty for the NP will be flexible to meet both staffing and administrative needs and may include evening, weekend, holidays and off-tour shifts.
