    Responsibilities

    The Liver Transplant RN Coordinator works under the direction of the attending surgeon or hepatologist, and within a multidisciplinary team that is responsible for the coordination of care for patients in outpatient settings, and for inpatient transplant education

    • The patient population includes patients being referred and evaluated for transplant, those currently listed for transplant, and patients post-transplant.
    • He/she develops a liaison between the Transplant Program and the many other disciplines that assist in caring for transplant patients.
    • The Liver Transplant RN Coordinator completes focused assessments of outpatients.
    • He/she orders laboratory tests, radiographic tests, and adjusts medications by following written protocol.
    • The RN provides documentation to meet transplant regulatory requirements.
    • The RN attends inpatient rounds and participates in and documents discharge education.
    • He/she also evaluates post-transplant patients in the Nursing Transplant Clinic following clinic protocols and consults with physician for situations not covered by these protocols.
    • The Liver Transplant RN Coordinator completes necessary documentation to maintain compliance with VA Headquarters regulations and the United Network for Organ Sharing {UNOS).
    • The Liver Transplant RN Coordinator provides "on-call" services during non-administrative hours for patients and medical/nursing staff at the hospital and home VA medical centers who need guidance concerning treatment of liver transplant patients, and for arranging and organizing transplant operations.
