    Responsibilities

    The Columbia VA Health Care System is recruiting for Licensed Practical Nurses (LPNs) for vacancies in the Primary Care Clinic (PCC) of Acute Service. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Patient Aligned Care Teamlet (Patient, Primary Care Provider, RN Care Manager and Administrative Associate) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient). Performs other related duties as required by Nurse Manager.

    Work Schedule: Monday - Friday, 7:30am to 4:00pm with evening and weekends as scheduled. Tour of duty is subject to change depending on agency needs.
