Duties include but are not limited to:
The Primary Care RN is assigned to a Primary Care Patient Aligned Care Team (PACT) and/or other area within the Ambulatory Care setting. The incumbent delivers safe and effective care to patients on a face to face basis and/or over the phone. Demonstrates advanced clinical knowledge in assessing, planning, implementing, documenting and evaluating acute/urgent care needs, as well as chronic disease management needs for assigned patients. Recognizes the socio-economic, age-related, gender-specific, cognitive, physical, & emotional needs of the adult, geriatric, and homeless patients, and incorporate into plan of care. Provides and facilitates evidence-based patient and family education. Maintains confidentiality of patient information. Adheres to safety, security, and health standards, as well as regulations and work practices. Communicates plan of care to patients and patient progress to provider and team. Uses available systems and resources to pro-actively apply the principles of Health Promotion and Disease Prevention (HPDP) to review, track and devise strategies to manage and improve the Health Effectiveness Data Sets (HEDIS) screening and wellness measures; some of the key measures include but not limited to mammography, cervical cancer screening, colorectal cancer, cardiovascular, opioid management, diabetes, osteoporosis, food insecurity, etc. Performs and documents day-2 post-discharge calls within required timeframes and complete all required notifications that are related to the notes. Reports work related illness/injuries. Completes all mandatory annual reviews and training. Demonstrates respect and communicates effectively with both internal and external customers. Establishes good time management, and performs other duties as assigned.
Work Schedule: Day, Monday-Friday, varies depending on department needs.

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
