    Work Schedule: Full-Time Monday-Friday 8:00am-4:30pm

    Incumbent is a Senior Social Worker in the Polytrauma System of Care (PCS) outpatient case management team. This includes working with Polytrauma Network Site (PNS) interdisciplinary team and the Transitional Care Managment (TCM) team at the VA Palo Alto Health Care System (VAPAHCS).

    The PSC outpatient case manager will connect Operation Enduring Freedom/Operation Iraqi Freedom (OEF/OIF) and recently transitioning Veterans who are registered with the VA PAHCS for mental health and healthcare services. This includes high quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers / agencies as needed by the client. This is accomplished in cooperation with other members of the TCM Programs or other interdisciplinary treatment teams as appropriate.

    The PSC outpatient case manager will work with the PNS team will coordinate the care of returning service members and Veterans who have experienced traumatic brain injury. The care may be received at any of the Health Care facilities and Community Based Outpatient Clinics {CBOCs} located within VISN 21, including VAPAHCS. The PNS team identifies, screens, assesses, evaluates and triages eligible returning combat service members/veterans to receive appropriate care throughout all the facilities within VISN 21 (major medical centers and the respective CBOCs). The PNS team works closely with all levels within the Polytrauma Rehabilitation System of Care ensuring the combat service member's/veteran's care is well coordinated, regardless of the level of care necessary or the locale of the treating facility within VISN 21.

    The major duties and responsibilities include, but are not limited to:

    • Incumbent is a professional social worker whose duties and responsibilities relate to the care management of severely ill and/or injured active duty service members and Veterans treated at the facility.
    • Care management responsibilities also include providing supportive services to families.
    • In addition, the incumbent assists service members and/or Veterans in coping with acute illness, chronic illness, combat stress, the residuals of traumatic brain injury (TBI), community adjustment, addictions, and other health and mental health problems.
