As a key member of the clinical leadership team, the Chief of PC is responsible for the delivery of Primary Care services to our Veterans at the Medical Center with direct line authority over the full-time and part-time staff delivering outpatient primary care in a Patient Aligned Care Team (PACT) model. He / She is a key member of the clinical leadership team and reports directly to the Chief of Staff. The incumbent ensures that high-quality patient care is delivered in an efficient and effective manner across all areas of responsibility. The incumbent should possess excellent leadership skills and be knowledgeable and proficient with VA Performance Measures and the principles of System Redesign including outpatient clinic access and hospital flow, as well as the Patient Aligned Care Team (PACT) and Patient Centered Care transformational efforts. The incumbent promotes an effective mix, coordination and support of clinical care by evaluating current and potential patient needs, clinical workload and resources, integration of PC services with all supporting services while highlighting the ability to foster an environment that encourages team care relationships focused on Veteran Centric care importance.
The incumbent will be responsible for the full implementation of the PACT model in all Primary Care clinics. The incumbent will foster an environment that builds relations with all support staff members of the PACT (i.e. nursing, nutrition, pharmacy, surgical, specialty medicine, radiology, lab, etc?) that ensures streamlined care is achieved. The Chief of Primary Care may also serve as a primary care provider for a relevant panel of veterans or possibly provide access clinics to Veterans based upon system/Veteran needs.
Work Schedule: Normal Schedule is Monday through Friday, day shift.

Travel Required: You may be expected to travel to VAMC Altoona, and other CBOCs as needed.
A limited amount of traveling to Pittsburgh VA health care system will be required for training purposes.
