    Social Worker positions are located within Behavioral Medicine Service in areas such as VA Supported Housing (HUD-VASH), Community Living Center (CLC), Patient Aligned Care Team (PACT), ACCESS Clinic, Inpatient/Outpatient Medicine, Substance Treatment and Recovery Program (STAR), etc.

    Duties include supporting the discharge plans from an acute medical ward, psychosocial assessment and family work. Incumbent participates on an interdisciplinary team and exercises independent judgment, decision making and timing of social work intervention of the psychosocial problems of patients. May serve on various medical center, mental health and/or community agency committees; integrates the cultural, social, economic and emotional aspects of illness into a comprehensive plan of treatment for specific patients; effectively relates to patients and significant others from various social, economic and ethnic backgrounds. Provides education related to long-term care planning, Advance Directives, community resources and VA resources.

    Work Schedule: Monday - Friday 8:00 a.m. to 4:30 p.m. work schedule, but schedule may be adjusted to meet the needs of the organization.
