    Summary

    The incumbent assigned to the Geriatrics and Extended Care (GEC) service line is a professional RN with experience in the acute hospital and extended care setting. The RN provides high-quality, evidence-based care for Veteran patients seeking care in the extended care setting. The incumbent is responsible to effectively plan for the Veterans' admission to the Community Living Center and/or Community Nursing Home Program and discharge to extended care or community setting.

    Responsibilities

    The GEC Care Manager RN communicates effectively with all providers of care, ancillary and support services, and external customers to facilitate efficient, patient-centric care. The incumbent performs assessments, prioritize care management duties, facilitates admissions and discharge planning. The incumbent serves as an expert consultant and collaborates with interdisciplinary team (IDT) to improve outcomes using evidence based practice. The incumbent documents in the electronic medical record using appropriate templates, protocols, SOP's guidelines and adheres to medical center policies.

    Work Schedule: Full-Time, 10 hours shifts, 80 hours per pay period with rotating weekends. Tour may change based on the needs of the organization.
