    This position is located within the Pharmacy Department at the Washington DC VA Medical Center. The Clinical Pharmacist Specialist Inpatient primary purpose is to support the general operations of Pharmacy to include non-formulary review, prescription/order processing, dispensing where required and patient consultation. The incumbent will help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in the assigned area.

    Work Schedule: Monday- Friday 7:30AM-4:00PM/occasional holidays and weekends.

    Responsibilities

    As a Clinical Pharmacist Specialist- Inpatient some of the duties are but not limited to:

    • To promote the optimal use of medications (including prevention of improper or uncontrolled use of medications)
    • To educate healthcare professionals, the patients and the public in pharmacotherapy.
    • Order laboratory and other tests to help determine efficacy and toxicity of medication therapy.
    • Interview and evaluate patients upon admission to the hospital.
    • Design a pharmaceutical care plan as appropriate and document the plan in the patient's electronic medical record.
    • Process and/or insure the processing of all discharge medications for the assigned patient care area.
    • Provide appropriate recommendations to providers regarding anti-thrombotictherapy for inpatients.
    • Coordinate the transfer of patients to the Outpatient Coumadin Clinic.
    • Provides consultative services to all professional staff regarding drug information and therapy.
    • Coordinate with the Antimicrobial Stewardship Clinical Pharmacist Specialist to ensure appropriate antibiotic use.
