    The Women's Specialty RN Care Coordinator uses the nursing process and evidence-based practice to collaborate with women's specialty care teams throughout VANCHCS in providing and coordinating care to women Veterans. The scope of this position includes care occurring in Gynecology, Women's Health Consult, Breast and Uro-Gynecology clinics, and EXCLUDES care occurring in Comprehensive Primary Care clinics. Responsibilities include practice management, triage of patient inquiries (including through secure messaging) with communication of care plans, patient appointment coordination, communication of normal and abnormal test results, and tracking of patients with known, significant conditions (e.g. gender-specific cancer, breast abnormalities, uro-gynecologic conditions, cervical dysplasia and adnexal masses) to optimize patient compliance with care plans. The Women's Specialty RN Care Coordinator will also coordinate obtaining outside medical records to provide continuity of care. The Women's Specialty RN Care Coordinator will serve as an advocate for patients, work effectively with team members, and will be proactive in enhancing education for self and the specialty care teams. Becomes competent in, and performs Conscious Sedation for minor procedures, as well as assisting with minor procedures that do not need sedation. Has knowledge of, and follows Sterile technique.
    VANCHCS specialty clinics occasionally refer women Veterans outside VA for specialized forms of care (e.g. cancer care and advanced pelvic organ prolapse surgery). The Women's Specialty RN Care Coordinator serves as a point of contact for referred patients and outside entities, and ensures that referred patients receive timely, high-quality care. The Women's Specialty RN Care Coordinator coordinates obtaining outside records for referred patients to provide continuity of care and to facilitate optimal coordination of care. Pregnancy care is managed by the VANCHCS Maternity Care Coordinator. Pregnancy-related duties for this position are limited to cross-coverage when the Maternity Care Coordinator is not present, and negotiated management of specific items at the request of the Maternity Care Coordinator.
    The Women's Specialty RN Care Coordinator works closely with the Radiology Service to care manage selected patients, to include those with BIRADS 4-6 mammography results, those requiring breast biopsy, and those requiring advanced breast imaging studies (e.g. breast MRI).

    VANCHCS serves a significant transgender Veteran population. The Women's Specialty RN Care Coordinator care manages selected transgender Veterans for selected items per provider request.The Women's Specialty RN Care Coordinator works with a variety of computer programs and data sources to accomplish work, including Computerized Patient Record System (CPRS), the Clinician Dashboard, VISTA data reports, care management software, and the Breast Cancer Registry (BCR). The incumbent possesses requisite proficiency with common office productivity software (e.g. Microsoft Word, Excel and PowerPoint). The Women's Specialty RN Care Coordinator attends role-appropriate meetings and participates in role-appropriate workgroups. The Women's Specialty RN Care Coordinator may participate in limited direct patient care activities according to their experience and scope of practice. Performs other role-appropriate duties as assigned. The Women's Specialty RN Care Coordinator collaborates with a wide variety of VANCHCS personnel in order to accomplish work, including the Chief of Gynecology, Gynecology providers and staff, the Maternity Care Coordinator, breast surgeons, the Women Veterans Program Manager, The Women's Health Medical Director, PACT RN Care Managers, and personnel in Radiology, Pharmacy, Lab/Pathology, and specialty clinics including Hematology-Oncology and Radiation Oncology.

    • Completes mandatory training, updates, memo/minutes updates and annual training updates.
    • Participates in Process Improvement activities/huddles
    • Maintain and stay up to date on assigned competencies including BLS/ACLS
    • Collects assess data based on patient signs, symptoms and history.
    • Provides health care advice to patients and family members seeking advice, utilizing the nursing process and triage protocol and policies.
    • Develops a plan of care using information from the patient, family, and or provider.
    • Completes required documentation assuring that it is reflective of patient needs.
    • Completes work assignments within established time frames; adapts to changes in or additions to work assignments including cross covering other specialty clinics whenever necessary; reprioritizes as necessary.
    • Maintains current emergency training and can instantly report responsibilities of position in the event of emergency.
