LVN is an active member of the interdisciplinary team. LVN complies with Department of Nursing/Unit specific and Medical center policies, procedures standards and protocols regarding direct care of patients, including medication administration, patient care planning, monitoring and documenting patient condition and response to treatment. LVN works under distant supervision of the RN. Has frequent contact and seeks clarification of assignment. Employee establishes work priorities and completes assignments in a timely manner with little to no supervision.Provides a wide range of personal care to patients in keeping with Department of Nursing Standards and with attention to preserving personal dignity, autonomy, religious preferences, cultural values and age appropriate needs.
Contributes to/modifies the patient care plan.
Organizes/prioritizes routine and non routine nursing care activities.
*Administers medications according to the "5 Rights".
*Appropriately uses protective and restraining devices as ordered.
Performs a wide range of wound care including irrigation, dressing changes, and care of various drains.
Performs ostomy care.
Implements post-operative/procedural exercises.
Implements use of urinary drainage devices and related care, completing accurate and timely intake and output.
Performs actions targeted to the maintenance/restoration of skin integrity and records patient response.
*Properly prepares oxygen dependent patients for transport.
*Discontinues peripheral IV lines as ordered.
Initiates patient/family interactions daily which provide emotional and psychological support.
*Maintains client/patient privacy.
*Observes, recognizes and reports significant or adverse changes in patients' condition in a timely fashion.
Administers/performs a variety of pain control measures and records results.
Administers all appropriate respiratory therapies, for example:
Oxygen saturation
Incentive spirometry
Aerosolized medications
Inhalers
Chest percussion/vibration
Postural drainage
Oxygen via nasal prongs or mask
Others as assigned18. Performs oral-pharyngeal and naso-tracheal suctioning.
19. Administers tube feedings.
20. Monitors gastric and tube feedings including checking residuals and flushing as directed.
21. Prepares patients for and monitors patients post-surgery/procedures.
22. Monitors neurological signs.
23. Implements seizure precautions.
24. Utilizes Doppler to check pulses.
25. Monitors chest tubes including type and amount of drainage.
26. Responds appropriately to emergency situations.
27. Implements structured patient activities.
28. Utilizes symptoms management principles. (for example: pain, anxiety, agitation, depression, schizophrenia, substance abuse).
29. Facilitates and/or leads therapeutic groups.
30. Applies and reads skin tests.
31. Assists with diagnostic and therapeutic procedures.
32. Sets clearly defined limits within the nurse/patient relationship.
33. Performs aspects of admissions/discharge procedures as directed.
34. Conducts skin assessment using the approved scale.
35. Recognizes manipulative patient behaviors and intervenes appropriately.
36. Demonstrates skills in intervening and following the protocol for managing potentially violent and/or suicidal patients.
37. Monitors medication side effects using the approved rating scales.
38. Demonstrates clear understanding of denial of rights, types of conservatorship, legal status; is sensitive to impact on patient; is able to justify rationale clearly and document.
39. Utilizes the approved model of Nursing care delivery with general supervision.
40. Demonstrates knowledge of and correct usage of medical and psychiatric terminology/diagnoses.
41. Demonstrates knowledge of and ability to explain legal holds to patients.
42. Complies with universal precaution and all infection control policies and procedures.
