VA Portland Health Care System (VAPORHCS) is a Level 1A major interdisciplinary teaching and tertiary facility with service-lines serving primary care, emergency medicine, medicine, surgery, specialty, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and extended care inpatients and outpatients throughout Oregon and Southwestern Washington. In addition to the two main divisions located in Portland, Oregon and Vancouver, Washington, VAPORHCS operates nine community-based outpatient/outreach clinics (CBOC) and is a consulting and referral center for many patient care specialties not offered at other VHA medical centers within VISN20. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. VAPORHCS operates a Community Resource and Referral Center (CRRC) which serves homeless Veterans in downtown Portland. The CRRC has a Homeless Patient Aligned Care Team (HPACT) co-located and offers primary care medical services to homeless and formerly homeless Veterans. Home-Based Primary Care nursing care is also provided throughout the VAPORHCS catchment area.
VAPORHCS received initial Magnet designation in May 2006, re-designation in December 2010, and again in October 2015.
The Psychiatric Mental Health Nurse Practitioner (PMHNP/MHC)/Family Nurse Practitioner (FNP) for the Residential Rehabilitation Treatment Program (RRTP) practices in the VA Portland Health Care System (VAPORHCS) Mental Health and Neurosciences Division RRTP. S/He will work as a member of a multidisciplinary team providing inpatient services to veterans with a variety of substance use disorders and comorbid medical and mental health conditions. The primary duty station is located in the RRTP program at the Vancouver Division site.
The Psychiatric Mental Health Nurse Practitioner (PMHNP/MHC)/Family Nurse Practitioner (FNP) for the Residential Rehabilitation Treatment Program (RRTP) will provide history and physical examinations and screenings for Veterans being admitted for the treatment of substance use disorders. S/He provides comprehensive primary, urgent, preventive and wellness care to Veterans admitted to RRTP. This includes performing histories and physicals, ordering and interpreting diagnostic and therapeutic test/studies, initiating consults and referrals, prescribing medications and supplies in accordance with VA Policy, VA formulary and State regulations. S/He will take an active role in formulating, initiating and evaluating health care teaching for RRTP patients and staff members. S/He will enter appropriate orders and follow through with referral to primary care, emergency services and specialty care as needed. The incumbent will screen for medical management of withdrawal and will be conversant with ambulatory detox protocols for alcohol and, when medically appropriate, will facilitate detox referral to either an inpatient admission within the VA or to one of VAPORHCS detox contract agencies. S/He will be conversant with VA/Department of Defense best practice guidelines for the pharmacological treatment of substance use disorders and will take referrals from the RRTP team members for pharmacotherapy for substance use disorders. S/He will consult with and support the physicians providing psychiatric and medical services external to RRTP and will provide general consultation regarding pharmacotherapy for substance use disorders to primary care physicians whose patients are being treated in this program.
The RRTP Nurse Practitioner guides, develops, and supports RRTP staff from a leadership perspective. S/He does this using professional standards of care, scientific evidence, and practice to evaluate programs and /or service activities, including development and review of RRTP policies and standard operating procedures.
