The Central Alabama Veteran Health Care System (CAVHCS) is a consolidated health care system that includes VA Medical Centers in Montgomery and Tuskegee, Alabama as well as Community Based Outpatient Clinics (CBOC) in Dothan, Alabama; Ft. The incumbent practices as a Licensed Practical Nurse in Mental Health Psychosocial Residential Rehabilitation Treatment Program in Mental Health and Behavioral Medicine. The LPN is responsible for the provision of nursing and rehabilitative services for residents and works in the residential setting in accordance to VHA Directive 1162.02. Duties include: implementation of approved policies and procedures; completion of required documentation in accordance with VHA, JCAHO, CARF regulatory and accreditation requirements; completes a comprehensive medical screening on all referrals to MH RRTP; assist the RN with ensuring completion of a comprehensive bio-psycho-social assessment through information gathering; participates in completion, documentation and implementation of psychosocial rehabilitative treatment plan by residents; functions as an integral member of the treatment team and assumes responsibility for assisting with completion of assessments for residents admitted to MH RRTP; serves as co-leader and case manager for an assigned panel of patients; coordinates and assist the physician/nurse practitioner/registered nurse with medical aspects of resident's care; makes notification and referrals to appropriate discipline(s) based on assessed needs; supports the mission and vision of outpatient programs e.g. Substance Abuse (SATP), Post Traumatic Stress Disorder (PTSD), Psychosocial Rehabilitation Resource Center (PRRC) and Veterans Industries (VI), Health Care for Homeless Veterans (HCHV), Mental Health Intensive Case Management (MHICM); provides education and counseling on disease prevention, health promotion, and medication management; ensures safe handling and ongoing compliance of residents with self-medication administration; coordinates medical appointments; conducts collection of urine drug screens and alcohol breath test; ensures safe environment and serves as lead during code green, fire and emergency response situations; initiates, participates and leads disciplinary proceedings for residents; participates in performance improvement initiatives; ensures completion of clinical practice guidelines as defined by VHA Office of Quality and Performance; initiates and participates in discharge planning. The incumbent is expected to provide specialized care to patients at all times, who may require complex treatments and who may be seriously ill. He/she is expected to be alert to changes in patient conditions and to report these observations to the registered nurse, as well as the need to modify patient care accordingly. He/she must adhere to ethical considerations by maintaining discretion in confidential matters while maintaining loyalty to the organization. The incumbent provides care to patients with a multiplicity of disease processes within the scope of licensed practical/vocational nurse license and provides for continuity of patient care.
Work Schedule: Rotating Shifts inclusive of weekends and holidays
Functional Statement Title: Licensed Vocational Nurse (LVN) GS 3-6.
Physical Requirements: This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures.
