VA North Texas Health Care System (VANTHCS) is the second busiest VA system in the nation and we are growing! Comprised of the Dallas VA Medical Center, Sam Rayburn Memorial Veterans Center in Bonham, Fort Worth Outpatient Clinic and Tyler VA Primary Care Clinic, VANTHCS is an integrated health care system with approximately 4,000 employees and over 1,600 volunteers and clinical trainees. VANTHCS also coordinates a Community Based Outpatient Clinic program throughout its 40-county primary service area to improve access to care for Veterans, 38 in North Texas and 2 in Southern Oklahoma. Through these four distinctive facilities and relationships with other agencies, VANTHCS offers a continuum of health care services to a Veteran population of approximately 500,000. Housed on a 84-acre campus, Dallas VA Medical Center is the corporate headquarters for VANTHCS and serves as the tertiary referral center. Patient Evaluations: Interprets medical record information, review therapy prescription, and conducts physical evaluation of patients in order to develop and implement a treatment plan in accordance with the physician's orders and patient's ability. Administers physical tests and assesses behavioral and emotional status for patients with severe or multiple diagnoses to assess treatment options. Uses effective interviewing techniques to obtain objective and factual information pertaining to the patient's interests and aptitudes.
Observes the complex patient during trial assignments to confirm initial assessment. Interviews patients with multiple diagnoses that require conventional interview approaches and treatments to devise effective methods and procedures. Establishes therapeutic goals that are meaningful and designs a treatment plan to meet such goals in the most efficient manner. Coordinates the overall treatment plan with the treatment team and the patient's family/caregiver to provide a program that focuses on treatment goals. Completes appropriately timed re-evaluations for the patients and modifies the goals based on the information gained by the re-evaluation.
Treatment Implementation: Coordinates patient schedule with other interdisciplinary team members. Identifies specific activities and routines to maintain or improve alternative ways of coping and, physical fitness or conditioning when impaired due to disease or disability. Develops therapeutic programs for various types of disabilities which include orthopedic conditions, amputation, muscle weakness, chronic cardiovascular and pulmonary disease, spinal cord injury, respiratory disorders, psychiatric diagnoses, dementia, etc. Utilizes various kinesiotherapeutic activities to maintain or achieve rehabilitation of the physical and/or behavioral-emotional impairments. Implements therapeutic exercises for strengthening, and teaches transfers activities and mobility training following prolonged bed rest, amputation, fractures, wounds, lower extremity joint replacement, etc. Provides the equipment, materials, instruction, encouragement, or counseling necessary to promote a positive and therapeutic activity. Functions as a team member of the multidisciplinary treatment team to provide total care delivery to the patient and to provide continuity of care by discussing progress with treatment team and, suggests adjustments to activities based on sound professional judgment. Modifies techniques and changes treatment interventions based on patient response to treatment in order to reach maximal outcomes. Modifies, redirects, and adjusts, treatment plans as indicated by each patient's response and progress in regards to each patient's physical, emotional and behavioral limitations, needs, skills, aptitudes, and interests. Reviews the patient's progress, introduces new skills or adjusts activities accordingly with patient response to treatment. Selects, adapts, and/or fits assistive devices in accordance with rehabilitation goals. May modify equipment and develop new devices in the realization of patient treatment goals. Manages difficult cases that may require unconventional applications to treatment and resourcefulness in collaboration with clinical specialist therapist or supervisory program manager. Evaluates and modifies treatment for patients with multiple or severe conditions. Participates in ward(s) rounds, IDT, JCAHO/CARF and any other relevant activities. Documents the treatment rendered and the patient's progress and response to treatment in the medical record. Documentation follows establish guidelines for timeliness and content per JCAHO, CMS, PM&RS, etc.
Work Schedule: Monday-Friday; 8:00 am- 4:30 pm, PRN weekend Coverage.
Physical Requirements:Moderate to heavy lifting, carrying, pushing and pulling is required to assist patients/caregivers and operate equipment. Use of fingers; both hands required; Walking (up to 3 hours); standing (up to 3 hours); Repeated bending (up to 1 hour); Near vision correctable to 13" to 16"; Hearing (aid permitted)]. The position demands significant physical stress and strain such as transferring patients, crouching and crawling on mats, and occasionally defending self against combative patients assigned to the incumbent. Sedentary aspects of this position require periods of sitting at a desk, utilizing the computer, keyboard, monitor, and phone, etc.
