The duties of the Formulary Program Manager include, but are not limited to the following:
Be a Drug Utilization Team (DUT) leader in one therapeutic area.
Provide leadership, direction, organization, planning, and implementation of the VHA
formulary at VAPORHCS.
Maintain a working knowledge of inpatient and outpatient processes to understand the impact of formulary decisions.
Participates in Professional Practice Review Committee for the department as assigned.
Reviews, evaluates and adjudicates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable plus documents per departmental procedure.When implementing formulary decisions, the incumbent must provide the following clinical duties and responsibilities:
1. Is directly involved in the design, implementation and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
2. Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates and documents findings with prescribers and provides appropriate alternatives to current treatment plans as needed.
3. Is able to apply knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.
4. Applies principles of clinical pharmacokinetics and pharmacodynamics and makes or recommends dose adjustments.
5. Performs medication reconciliation, including but not limited to obtaining information from patients and summarizes significant findings in the medical record and/or to the provider.
6. Age/gender specific monitoring of therapy shall include review for polypharmacy,
dose adjustments, and appropriateness of therapy.
7. Reviews and verifies medication orders to be administered to patients
for appropriateness and to reduce potential risk of adverse drug event.
8. Monitors and documents drug errors, adverse drug reactions, allergies,
and patient compliance issues per facility procedures.

Physical Requirements: Incumbent may be required to do moderate to heavy lifting (15- 45 pounds and above), moderate carrying of 15- 44 lbs, reaching above shoulders, use of fingers and both hands is required. Walking and standing 4-8 hours a day. Ability for rapid mental and muscular coordinating simultaneously. Will be required to pass a vision test. Mental and emotional stability required.
