The Registered Nurse Home Based Primary Care (HBPC) position is located in the Home Based Primary Care section of the Primary Care Service at Hartshorne, OK of the Eastern Oklahoma VA Health Care System (EOVAHCS), Muskogee, OK. The Home Based Primary Care Staff Nurse is a registered nurse who is a graduate of a professional nursing program approved by the appropriate state-accrediting agency and is BLS certified. The Home Based Primary Care Staff Nurse has advanced clinical knowledge, more than 2 years of RN experience, and competent to function in a variety of roles in the home and clinical setting. S(H)e provides primary care assessment, care and education to the Veteran in the home setting, participates on hospital committees and in educational programs. The Home Based Primary Care Staff Nurse is accountable for utilizing critical thinking skills and sound clinical/supervisory judgment to ensure positive patient outcomes; for demonstrating ethical practice; and for participating performance improvement activities to improve the quality of care and ensure performance measures are met and/or exceeded. The Home Based Primary Care Staff Nurse is directly responsible to the Community and Home Based Care Nurse Manager and an active member of Primary Care Services. Travel is required.
Work Schedule: Monday through Friday, 8:00 am - 4:30 pm (Telework, Flexible tour of duty may be optional)
50% or less - You may be expected to travel for this position.
