The Mental Health NP is responsible for providing and coordinating care for all aspects of patient care including preventive, urgent and routine care. Many of these patients are high risk with complex medical conditions and co-morbidities. The NP performs the following duties:
-Completes comprehensive psychiatric evaluations, history and physical exams, medication management, orders and interprets diagnostic studies, orders medical/surgical/specialty care consultations as needed and prescribes appropriate medication.
-Develops, evaluates and manages mental health treatment plan throughout all stages: prevention and wellness, acute and chronic illnesses, focusing on complex and higher level management patients.
-Demonstrates expertise in diagnosing and treating acutely mentally ill multi-symptomatic patients and consistently makes and implements sound and reasonable decisions demonstrating responsibility for the outcome.
-Provides care management that includes addressing view alerts, clinical reminders, preventive care needs, medication reconciliation, and refers to other team members as appropriate.
-Identifies high-risk patients for hospitalization not limited to patients with suicidal flags, behavioral flags, OEF/OIF/OND, chronic pain, substance users, etc.
-Reviews and follows-up on diagnostic test results and communicates findings to patients.
-Initiates consults based on knowledge of pre-requisites with appropriate work-up completed prior to request, follows the treatment plan of care as recommended and discusses plan with specialists as needed.
