The primary responsibility of the Clinical Pharmacy Specialist (CPS) is the provision of evidence-based clinical pharmacy care in the management of Hepatitis C at Northern Indiana Healthcare System. A therapeutic responsibility of the position is to work with other pharmacists and practitioners to provide comprehensive clinical and educational services, precept residents and students on clinical rotation, and perform other duties as assigned. The incumbent possesses advanced training and expertise for managing hepatitis C treatment and works cooperatively with providers to manage patients and coordinate care.
An additional therapeutic responsibility of the position is coordination and provision of clinical pharmacy services to patients in the Patient Aligned Care Team (PACT). The pharmacist monitors and manages drug therapy for patients referred with chronic disease states within scope of practice. The incumbent provides pharmaceutical care utilizing an advanced in-depth knowledge of current literature and contemporary practice of medications. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The scope of this practice includes patient teaching, and consultation, and in association with or in support of pharmacists, providers, nurses, dietitians, and other health care practitioners.
Work Schedule: Monday - Friday 7:45 - 4:30.

BASIC REQUIREMENTS:
(1) Proficient in written and spoken English.
(2) U.S.Citizen. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
(3) Graduate
of an Accreditation Council for Pharmacy Education (ACPE) accredited College or
School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or
a Doctor of Pharmacy (Pharm.D.) degree.
NOTE: Prior to 2005 ACPE
accredited both baccalaureate and Doctor of Pharmacy terminal degree program.
Today the sole degree is Doctor of Pharmacy.
(4) Graduates of foreign pharmacy degree programs meet the educational
requirement if the graduate is able to provide proof of achieving the Foreign
Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes
passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the
Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
(5) Residency and Fellowship Training Preferred. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year for year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A residency or fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) or American Society of Health-System Pharmacists (ASHP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).
Licensure:
Full, current and unrestricted license to practice pharmacy in a State,
Territory, Commonwealth of the United States (i.e., Puerto Rico), or the
District of Columbia. The pharmacist must maintain current registration if this
is a requirement for maintaining full, current, and unrestricted licensure. A
pharmacist who has, or has ever had, any license(s) revoked, suspended, denied,
restricted, limited, or issued/placed in a probationary status may be appointed
only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3,
section B, paragraph 16.
(1)Exception.
Non-licensed pharmacists who otherwise meet the eligibility requirements may be
given a temporary appointment at the entry level as a Graduate Pharmacist under
the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive
the requirement of licensure for a period not to exceed 2 years for a
pharmacist that provides care under the supervision of a licensed pharmacist.
For grade levels above the GS-11, the candidate must be licensed. For those grades that require licensure (GS-12 and above), the incumbent must hold a full, current, and unrestricted license in a State, Territory, Commonwealth, or the District of Columbia.

Education and Experience:
1.In addition to the basic requirements, completion of 1 year of professional experience comparable to the next lower level OR
2.Completion of an ACPE-accredited Pharm.D. program.
Assignments. Pharmacists at this grade level are independent licensed practitioners and are considered to be above the full performance level.
At the GS-13 level the following KSAs are required:
1.Ability to
communicate orally and in writing to persuade and influence clinical and
management decisions.
2.Expert understanding of regulatory and quality standards for their program
area.
3.Ability to solve problems, coordinate and organize responsibilities to
maximize outcomes in their program area or area of clinical expertise.
4.Expert knowledge of a specialized area of clinical pharmacy practice or
specialty area of pharmacy.
5.Advanced skill in monitoring and assessing the outcome of drug therapies,
including physical assessment and interpretation of laboratory and other
diagnostic parameters.
