The Patient Aligned Care Team (PACT) LPN is responsible and accountable for screening /checking in and providing direct patient care in the Primary Care Clinics under the supervision of a Registered Nurse and/or Providers. The LPN is responsible for nursing care provision to Veterans scheduled for treatment of illness and preventive health care alongside your PACT team. The LPN prepares and administers prescribed medications or immunizations
and collects & prepares specimens for lab analysis. The LPN assists providers during diagnostic,
therapeutic and/or minor surgical procedures by preparing necessary supplies & equipment:
Strong medical/surgical background to perform LPN duties in a PACT nursing clinic
Demonstrate strong team work skills
Excellent Communication skills, both verbal and written
Excellent problem solving skills
Demonstrate excellence in nursing care
Requirements include: Basic Life Support, Active Nursing License, and experience in primary care
Strong computer background is preferred.
Work Schedule: Standard tour of duty 5 days week, 7:30 am-4:00 pm - extended clinic hours may be included on a rotating basis dependent upon PACT schedule and clinic needs.
Location: This position is located in the Outpatient Clinic, Primary Care Service Line Hot Springs, SD.
Relocation Expenses: Relocation expenses are not authorized for this position.
