The Nurse Practitioner, Home Based Primary Care (HBPC), is a registered, professional nurse practitioner (APRN) who provides care at home to a clinically and psychosocially complex population of chronically ill or severly disabled veteras of the Pacific Islands Health Care System (PIHCS).  This position is assigned to the Maui Community Based Outpatient Clinic (CBOC).

The Home Based Primary Care NP is responsible for assessment, planning, coordinated provisions, and evalulation of individual treatment plans for patients receiving care from the HBPC program of the PIHCS.  The NP is an important member of the interdisciplinary HBPC team providing care to patients admitted to the program.

  • Travel will be required to go to the Neighboring Islands of Lanai and Molokai, as well as around the Island of Maui
