Provides a full range of nursing care to Veteran Residents on a Community Living Center unit with a variety of physical and or/behavioral problems, psychosocial and medical care needs generally consisting of rehabilitation/restorative therapy, palliative or respite care, wound care, dementia care, IV antibiotic treatments and other long term skilled nursing care. Conducts a physical assessment on patients, determines their nursing needs, and devises an individualized plan of care. Documents vital signs, significant, and reactions. Observes behavior of patient to determine psychological state and provides emotional support. Participates in continuing education actives. Administers medications utilizing BCMA and PYXIS. Functions as a Charge Nurse or team leader. Completes assigned MDS’s. Develops and updates nursing care plans. Directs patient care activities and collaborates with other health care members. Directs and monitors staffing assignments and delegates care coordination based on resident needs and staff skill/competency level ensuring all aspects of the Veteran resident’s plan of care are met.

This position is located within Geriatrics and Extended Care Line Community Living Centers (2&3), at the VA Medical Center, Bath, New York.

 

TOUR OF DUTY:

Nights: 12:00 am to 8:00 am.

Evenings: 3:30 pm to 12:00 am

BASIC REQUIREMENTS:  

  • U.S. citizenship.  
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). 
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
  • Graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration and the possession of a Certificate from the Commission on Graduates of Foreign Nursing Schools (CGFNS) will meet the requirement of graduation from an approved school of professional nursing.
  • Must be proficient in spoken and written English.

GRADE DETERMINATIONS:

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience. 
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

Credentialing & Privileging: Any registered, and certified health care providers appointed to a Veterans Health Administration Medical Center must enter their employment history, education history and all licensure/certification/registration information through the internet-based credentialing system (VetPro). VetPro ensures the consistency of the credentialing process for all employees and provides a reliable electronic data bank of health care provider credentials. This system is used nationwide by all VHA healthcare facilities. Employment commencement dates will be contingent upon the completion and approval of the VetPro credentialing/verification process.

 

Salary will be determined by a Nurse Professional Standards Board based on educaiton, experience and qualification standards.
