This vacancy is for Home Based Primary Care Medical Director (Physician-Geriatrics) at the Albany, VAMC. This vacancy will be used to fill one full-time permanent appointment. The tour of duty will be day shift with on call, off-tour coverage on a rotational basis.

The Albany VA Medical Center is seeking a physician board certified in Internal Medicine or Family Practice to serve as the Home Based Primary Care Medical Director. The Albany VA is located in the beautiful Capital District of NY which boasts variety of cultural offerings and outdoor activities, excellent quality of life, good cost of living and proximity to larger metropolitan cities including NYC, Boston and Montreal.
The full-time Medical Director for the Home Based Primary Care Program position integrates direct care and clinical leadership of the Home Based Primary Care (HBPC) Program and is designed to foster innovative models of care for a broad range of patients. The applicant selected will provide leadership as the HBPC Medical Director, working collaboratively with the HBPC Program Director and nurse manager to provide clinical guidance for the mid-level providers, clinical support and teaching for the interdisciplinary team and provide comprehensive primary care within the HBPC Patient Aligned Care Team (PACT) HBPC model. The physician assists in teaching resident physicians or students.

- Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.
- Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
- Must be proficient in spoken and written English
- Board Certified in Internal Medicine or Family Practice (required)
- Board Certified or Board Eligible in Geriatrics (preferred)
- Possess BLS certification
- Current and valid Driver's License

PHYSICAL REQUIREMENTS: The physician should be physically and mentally capable to perform the duties of the position. Physical activities include sitting, standing (up to 2 hours) walking (up to 2 hours), heavy lifting (45 pounds or over), reaching above the shoulder, and repeated bending.
