The Louis Stokes Cleveland VA Medical Center is a 673-bed tertiary healthcare facility which provides care to over 100,000 veterans in northern Ohio. We provide care through our main medical facility at the Wade Park campus, 13 community based outpatient clinics, and 3 vet centers. The medical center has been recognized for more programs of excellence than any other Veterans Affairs Medical Center. The Louis Stokes Cleveland Veterans Affairs Medical Center is recruiting for a Registered Nurse (Collaborative Care Case Manager). The Registered Nurse (Collaborative Care Case Manager) will function within the Emergency Department in the Patient Transfer Center for Patient Care Administrative Services. The Registered Nurse (Collaborative Care Case Manager) duties include:

  • Utilizing data to develop a patient-centered interdisciplinary discharge plan of care.
  • Working collaboratively with diagnostic and ancillary services related to tests, procedures, and appointments to improve patient flow and throughput.
  • Exhibiting excellent relationship skills that facilitate communicating patients' needs, appropriate level of care, and barriers related to throughput with all members of the healthcare team.
  • Leading interdisciplinary team meetings to discuss planned or anticipated discharges within 24-48 hours.
  • Facilitating and coordinating the discharge plan by: placing travel consults, scheduling discharge appointments, order appropriate home going supplies, placing prosthetic consults, monitoring clinical reminders for the pneumovax vaccine, influenza vaccine, PPD's and other reminders appropriate to the facilitation of the discharge plan.
  • Conducting daily utilization management patient reviews to determine appropriateness of admissions and continued stays using the National Utilization Management Integration (NUMI) program, InterQual criteria.
  • Acting as a resource coordinator by notifying appropriate nursing staff and other health care team members regarding the discharge plan.
  • Being administratively responsible to the Nurse Manager; Patient Transfer Center and works collaboratively with the Chief of the Patient Transfer Center, Chief of Patient Care Administrative Services, attending Physicians, house staff (residents and interns) mid-level providers and staff at all levels throughout the Medical center and outpatient care system.

Work schedule:  Monday - Friday, 10:00am - 6:30pm

In order to qualify for this position you must meet the following requirements:

  • Citizen of the United States.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
  • Current, full, active, and unrestricted registration as a graduate professional nurse in any State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.  In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows:

  • Practice: practice, ethics and resource utilization.
  • Professional Development: performance, education/career development.
  • Collaboration: collaboration, collegiality.
  • Scientific Inquiry: quality of care, research.

Education/Experience Requirements:

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelor's degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
  • Emergency Room experience is preferred.
