The Licensed Practical Nurse works under the direction of an RN and is part of the Patient Aligned Care Team (PACT) with a focus on patient centered care delivery. The LPN will function at the top of his or her license. The Telehealth (TH) LPN is responsible for observing, recording and reporting changes in the patient’s condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. The TH LPN demonstrates active collaboration, communication and problem solving with Core Team (Patient, Primary Care Provider, RN Care Coordinator and Clerk) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). The LPN is responsible for actively participating as part of a high functioning team in meeting the PACT initiatives and outcomes. The Telemedicine LPN is responsible for coordinating telemedicine clinics at the CBOC with the specialty providers consulted at various VA resources within VISN 15. The TH LPN maintains competency in set-up, use, and maintenance of the telemedicine equipment and serves as a resource to the primary care staff in use of telemedicine equipment.

The LPN will be assigned to assist other PACT teams as needed at the Belton Community Based Outpatient Clinic (CBOC) or at the KCVA campus. This position is supervised by the Nurse Manager of the assigned CBOC.The current work schedule is eight hour days, Monday through Friday; however, there is potential for some requirements for coverage on the weekends.  The individual in this position is supervised by the nurse manager of the clinic.

First area of consideration will be given to permanent Kansas City VAMC employees.  Internal and External Candidates will be considered.    

Note:  The full performance level of this vacancy is GS-6.  The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6.  The Professional Standards Board (a peer-review group) will review the selected candidate’s education and experience and recommend the grade and step at which the candidate will be placed.  The salary is based on the grade and step approved for the selected candidate.

·         Maintains competency in set-up, use, and maintenance of the telemedicine equipment

·         Serves as a resource to the primary care staff in use of telemedicine equipment.

·         Administers medication to include IV medication.

·         Performs delegated procedures and treatments including by not limited to wound care, foley insertions, EKG

·         Appropriately  documents all clinical interactions as part of visit or phone or secure messaging

·         Performs phone Management duties that include  follow up calls to patients, calls from patients and responding within scope or facilitating other team involvement, post hospital call and ED calls, coordination with outside providers and entities; pre- clinic visit calls to initiate screenings and obtain patients’ needs for team planning prior to visit.

·         Provides patient education including but not limited to glucometer, b/p machines, wound care, catheter care

·         Assists the team in meeting PACT outcome measure which may also include Data collection and reports

·         Demonstrates professional behavior (i.e., respect others, collegiality, ability to accept constructive criticism), effective communication and interpersonal skills.

·         Demonstrates the knowledge and ability to function as part of a high functioning team and proactively participates in all process improvement efforts of the team.

·         Demonstrates knowledge and skills in customer service delivery and patient advocacy
