NOTE: This position is in the U.S. Public Health Service (USPHS) Commissioned Corps and you will be referred as a Commissioned Corps personnel and not as a candidate to a permanent GS career or career-conditional appointment. This USPHS position is being filled at the rank of Captain (O-6).

This is a key position within the Immigration and Customs Enforcement (ICE). The ICE Health Service Corps (IHSC) exists within the organizational structure of the U.S. Immigration and Customs Enforcement, Enforcement and Removal Operations (ERO) under the supervision of the Department of Homeland Security. The United States Public Health Service Commissioned Corps Officers, civil service staff and contract employees comprise the health care professional working together to provide quality health care services.  

The incumbent serves as the Medical Officer (Clinical Director) for one of the health care facilities located across the nation for the ICE Health Service Corps (IHSC). These sites provide medical care to detainees in Immigration and Custom Enforcement (ICE) custody. IHSC serves as the medical experts for ICE.

MISSION:The ICE Health Service Corps protects America by providing health care and public health services in support of immigration and customs law enforcement. This is accomplished through maintaining a committed, effective and efficient workforce who provides evidenced based appropriate health care in a cost effective manner.

COMPETENCIES:

  1. Must be board certified in family practice, internal medicine, or related medical specialty.

Duties

DUTIES AND RESPONSIBILITIES: 

  1. Interpretation and translation activities within the scope of the employees’ duties.
  2. Assess these employees’ language abilities to ensure their competency to perform language services for particular duties; and describe plans for enhancing employees’ language abilities.
  3. Provides and oversees care appropriate to patient needs, severity of disease, condition, impairment, or disability within the scope of care provided in the detention setting (short stay unit care vs. ambulatory care) and within the mission, vision, and goals of the organization.
  4. Assures appropriate diagnostic services are available, utilized and monitored.
  5. Serves as the on-site medical authority within the scope of services provided by IHSC.
  6. Responsible for developing and coordinating all aspects of healthcare delivery at his/her assigned facility.
  7. Seeks guidance from the IHSC Medical Director, Deputy Director for Clinical Services or other senior leadership as appropriate.
  8. Serves as chairperson of local performance improvement and pharmacy therapeutics committee.
  9. Coordinates with the Health Service Administrator (HSA) to prepare for the accreditation and survey process for: The Joint Commission (TJC)· National Commission on Correctional Health Care (NCCHC)· American Correctional Association (ACA)
  10. Supervises mid-level providers (NPs and/or PAs). In the event that the on site HSA is not a clinician, then the CD will also have clinical supervisory responsibility over the nursing staff and pharmacy.
  11. Reviews and signs the medical charts, EKGs, Chest X-rays and other medical documents prepared by the mid-level providers.
  12. Provides program/clinical expertise for the development of site visit protocols and reporting requirements for management of primary care programs for the detention facilities.
  13. Must keep abreast of emerging treatment regimens, medical literature, and current medical knowledge and practices.
  14. Provides guidance and serves as a subject matter expert for emerging evidence based practices and assists in the development of future medical policies and standard operating procedures.
  15. Assures that high quality clinical standards are practiced and maintained at the medical facility.
  16. Available to provide on-site medical support to other IHSC facilities or headquarters through a temporary duty assignment (TDY).
  17. Assure clinical knowledge, skills, and responsibilities of the medical staff.
  18. Prepares presentations for professional, academic and special interest group meetings or conferences.
  19. Serves as an authority on national and international correctional healthcare trends and healthcare service delivery issues.
  20. Collaborates with the Medical Director and other senior staff for the development of budget proposals and programmatic plans.
  21. Analyzes healthcare trends related to special population groups and assists with developing and implementing policies and guidelines to improve healthcare services for special populations.
  22. Provides leadership and guidance to professional and technical staff in areas of program planning, development, and evaluation. Develops and implements a plan for evaluating program efficiency and effectiveness.
  23. Evaluates program goals and objectives, priorities, and activities in terms of current operating activities, program progress, and changing health needs and demands. Develops and monitors an implementation plan in support of overall program goals.
  24. At the request of the Director, represents the IHSC to ICE/DRO, Agencies and Offices within the Department of Homeland Security, Regional Offices, Health Human Services (HHS), other Federal and non-Federal agencies, grantees, and advocacy organizations, to ensure a coordinated effort in improving healthcare for the detainees.
  25. Collaborates with representatives of State and local health agencies, as well as professional and community based organizations concerned with improving primary care.
  26. Supports Agency-wide programs related to healthcare services such as:Reviews and analyzes data.Develops strategies, analytical models, and methodologies.Provides assistance and advice on healthcare issues.Supports Senior Consultants and other Specialists.Analyzes policies and, provides guidance implementation to assure consistent interpretation and application.Assists in identifying successful nursing and medical care strategies and projects that prove particularly successful for information and implementation.Reviews strategy reports on progress toward the achievement of goals.

  27. Every participant is required to engage in the full-time outpatient clinical practice of the profession for which he or she was awarded an NHSC Scholarship contract.
  28. A full-time practice is defined as a minimum of 40 hours per week. The 40 hours per week may be compressed into no less than 4 days per week, with no more than 12 hours of work to be performed in any 24-hour period. Time spent in "on-call" status will not count toward the 40-hour week. Hours worked over the required 40 hours per week will not be applied to any other workweek.
  29. No more than 7 weeks (35 workdays) per service year can be spent away from the practice for vacation, holidays, continuing professional education, illness, or any other reason. For absences of greater than 7 weeks in a service year, the participant must request a suspension. A break in service will extend the service commitment end date.

  30. A full-time clinical practice is defined as follows:
  • For all health professionals, except obstetrician/gynecologist (OB/GYN) physicians, family medicine physicians who practice obstetrics on a regular basis, certified nurse midwives, and psychiatrists, at least 32 of the minimum 40 hours per week must be spent providing direct outpatient care.
  • These services must be conducted during normally scheduled clinic hours in an ambulatory care setting in the office(s) specified in the PPA or PPO Agreement. The remaining hours must be spent providing inpatient care to patients of that clinic (as appropriate to meet the needs of the patients and to assure continuity of care) and/or in practice-related administrative activities.
