A full-time staff RN position is open in Home Care Services - Community Health Nursing.  The selectee provides support for the extended care programs including the Community Nursing Home program and the Adult Day Healthcare program through routine patient follow-up visits in the community, new and yearly inspections, and follow-up on care issues in the facility. The selectee also provides support to the Homemaker/Home Health Aide program through assessment, referral, quality monitoring, and tracking of patient census.  The selectee is a member of the Community Health Nursing Team and provides community health support to the continuum of care of the Medical Center through evaluation, consultation, case management, and discharge planning to meet home care needs of inpatient and outpatients through referral and coordination with community home care agencies.  This position reports to the Associate Chief Nurse, Home Community Based Services.  Tour of duty will be eight-hour day shifts with weekends and Federal holidays off.    

Relocation expenses are not authorized.

BASIC REQUIREMENTS:

CITIZENSHIP: Citizen of the United States. 

EDUCATION: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). 

REGISTRATION/LICENSURECurrent, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.  In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

Must be proficient in spoken and written English.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.   Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are.  Education/Experience requirements: 

  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
  • Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience. 
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience. 
  • Minimum Qualifications: 

    1) Completion of an NLN-accredited nursing program.

    2) Meets requirements for RN licensure or eligible for RN Licensure

    3) Documented evidence of satisfactory performance and dependability preferred.

    4) Demonstrated ability to perform duties in accordance with Joint Commission Standards and Nursing Standardsa of Care.

    5) Ability to easily establish and maintain effective interpersonal relationships and communication with physicians, staff, patients and their significant others.

    6) Working knowledge of the application of the nursing process in the management of medical/oncology patients.

    7) Ability to communicate effectively both verbally and in writing.

    8) Computer literacy is essential as staff utilize an electronic medical record system.

    9) BSN preferred.

    10) Two years of recent clinical nursing experience with documented evidence of good clinical, assessment and problem solving skills.

    11) Current Arizona's driver license is required.

    12) Microsoft Word and Excel is required.

    PHYSICAL REQUIREMENTS: Heavy lifting (45lbs and over); heavy carrying (45lbs and over); straight pulling (6 hours); pushing (6 to 10 hours); kneeling (1 hour); repeated bending (4 hours); both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13” to 16”; far vision correctable in one eye to 20/20 and 20/40 in the other eye; ability to distinguish shades of color; heading (aid permitted).
