Works under the clinical direction of a RN and is responsible for observing, recording and reporting changes in the patient’s condition. 

 

Makes observations and thoroughly documents care and the patient’s response.

Seeks assistance and guidance by informing RN or medical provider of changes in the patient’s condition or urgent patient care situations requiring higher-level intervention. 

Performs, reports, and records assigned nursing care in the Primary Care Clinics including, but not limited to: oxygen therapy, medication administration, clinical reminders with appropriate consults and alerts as indicated, checks expiration dates of meds and supplies, assists with minor procedures such as obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy, and monitoring patient during the procedure

Provides patients and their family health education that includes self-management, prevention and wellness.

Works within an interdisciplinary team environment.

Performs other related duties as assigned.

BASIC REQUIREMENTS:   

  • U.S. citizenship.  Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.   
  • (1) Graduate of an approved practical or vocation nursing school or health care education or training in the military service that is accepted by a State licensing body.
     
    (2) Current, unrestricted State license as a licensed practical or vocation nurse.

Preferred Experience: Three years qualifying LPN experience.

GRADE DETERMINATIONS:
Salary is determined by the Licensed Pracitcal Nurse Professional Standards Board (PSB).

Physical Requirements: Licensed Practical Nurse Functional Requirements: Heavy Lifting (45 lbs and over); Straight Pulling (up to 8 hours); Pushing (8 hours); Reaching above shoulder; Use of fingers; Both hands required; Walking (up to 8 hours); Standing (up to 8 hours); Kneeling; Repeated bending (up to 8 hours); Both legs required; Ability for rapid mental and muscular coordination simultaneously; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Hearing (aid permitted).
