The Clinical Pharmacy Specialist (CPS)/Anticoagulation Services serves as an outpatient provider with prescribing privileges as outlined in the individuals Scope of Practice for Clinical Pharmacy Specialist in anticoagulation. The clinical pharmacy specialist functions at the highest level of clinical practice and works independently under their scope of practice to directly care for patients. Normal clinic hours are Monday-Friday from 8AM - 4:30PM, but hours may change to accommodate veterans clinic access. Pharmacy Residency Training accredited by the American Society of Health-System Pharmacists (ASHP) is preferred.
